Transportation Request Form

Please complete the form below if you are seeking transportation for your student.  If transportation can be provided, priority will be given to students who reside in areas where buses are currently available. Students will be picked up at a nearby corner or school site.

Your student’s District ID is required to complete this form.  If you do not have this information, you can find it from the following sources: LAUSD Parent Portal, report cards, standardized student testing reports, and other correspondence received from LAUSD.

Formulario Para Solicitar Transportación

Por favor complete el formulario a continuación si está solicitando transportación para su estudiante.  Si transportación se le puede proveér, se le dara prioridad a los estudiantes que reciden en las areas donde actualmente hay autobuses disponibles.  Los estudiantes seran recojidos en la esquina o escuela mas cercana.

Se requiere el número de identificación del distrito de su estudiante para completar este formulario. Si no tiene esta información, puede encontrarla en las siguientes fuentes: Portal para padres de LAUSD (Parent Portal), reporte de calificaciones, informes de pruebas estandarizadas y otra correspondencia recibida de LAUSD.
